Document Center: Add Optional Fields to Waivers

The Waiver System lets you add extra signer details to digital waivers, such as email, phone, and parent or guardian information. These fields can be enabled in Optional Fields, shown on specific waiver templates, and used to make signed waiver records easier to search.


Prerequisites

  • Permission to access Optional Fields.
  • Permission to access Document Center and manage Waiver Templates.
  • At least one waiver template created.
  • If your account uses contact creation from signed waivers, make sure your waiver collects the contact details you want to save.

Steps: Enable and Use Waiver Optional Fields

  1. Open Optional Fields
    Go to Settings -> Optional Fields.
  2. Enable the waiver fields you want available
    Activate the waiver-related fields you want to use. The new fields include Email, Parent/Guardian Name, and Phone.
  3. Save your Optional Fields changes
    Fields must be active before they can be used on a waiver template.
  4. Open Waiver Templates
    Go to Settings -> Document Center -> Waivers.
  5. Create or edit a waiver template
    Open an existing template or add a new one.
  6. Select which fields appear on that waiver
    Choose the optional fields you want shown on the template. You can configure this per waiver template.
  7. Save the waiver template
    Your selected fields will now appear when that waiver is used.
  8. Send or assign the waiver
    Add the waiver to a lead/event and send the secure waiver link as usual.
  9. Review signed waiver responses
    After the waiver is signed, the entered field values are saved with the signed waiver record and can be searched in Signed Waivers.

Feature Options & Parameters

  • Optional Fields activation: Email, Parent/Guardian Name, and Phone must be activated in Optional Fields before they can be used on a waiver.
  • Per-template field selection: Each waiver template can show a different set of fields.

Use Cases

  • Collect parent or guardian details for participants who need adult approval.
  • Capture signer email and phone on the waiver so the information can be searched later or used to create a contact record.

Troubleshooting & FAQs

  • I do not see Email, Parent/Guardian Name, or Phone on the waiver.
    First activate the fields in Optional Fields. Then edit the waiver template and make sure those fields are selected there as well.
  • I activated the field, but it still is not available in the template.
    Save the Optional Fields change, then reopen the waiver template and save it again.
  • Signed waiver data is not easy to find.
    Go to Signed Waivers and search for the signer or waiver record there.
  • A contact record was not created from the waiver.
    Make sure the waiver collected the needed contact details and that your account is configured to create contacts from signed waivers.
  • Can I show different fields on different waivers?
    Yes. Field display is configured per waiver template.
