Survey Alert Not Working

We don't seem to be receiving alerts anymore when a customer survey is completed? This used to work fine.
The alert is active and set to the correct email address.
Example leads: 6506685, 6622327, 6640851

We are not seeing a pattern just yet. We released a change we think may have solved it, but we are monitoring. Thanks!
