Signable doc issue

We just had lead #60573907 sign the customer pickup signable document. The link on the app took them to the appropriate page and everything worked. BUT when I went to look at the doc on the lead, the doc name says "default," and it's the default IO contract template with the signature my default one is not there.
A little more context. If I uncheck the default box of the customer pick up form, then it shows the IO default contract. on the link. If i go to each lead and set the signable document manually as customer pick up doc, then everything works as it should
