Intermittent Last Name Required Error on Quote Form

Hello,
We’re having an intermittent issue affecting some customers (I’ve also been able to trigger it once myself).
When a customer fills in the quote form and clicks Submit, it sometimes throws the error:
“Please enter last name”
This happens even when the Name field is already filled in (e.g. John Doe). If the customer clicks submit again, the quote usually goes through without any changes.
I’ve been unable to find anywhere to disable the “last name required” validation. Are you able to make the Last Name field not required for submitting a quote, or advise where this validation can be adjusted?

We have been conducting tests but have not been able to replicate this issue on our end.
Would you be able to share specific examples of leads where this occurred, or possibly provide a video demonstrating what you or the customer is doing when these leads are received?
