IO not logging calls outgoing when calling out to customer directly from IO mobile app.

We don't have an example to work with here, but it sounds like the phone itself might be dialing out directly instead of going through IO Phone. If you call out of the app, you should see that you received a call from your IO Phone number in your most recent calls, NOT that you made an outbound call to the customer's phone number. Is that the case?
