Custom questions and form fields

I would like to add questions for people to answer that can add a fee based on their answer. I've added a pic for reference. Based on what I have already found it looks like this would need to be coded. Please let me know what I need to provide to get this done.

This can be added by creating optional fields within Settings-->Optional Fields.
