Can you add a check box during checkout for parks

Is there a way to add a required yes/no check box for parks in the Event Details part of the checkout?
No would have everything remain the same to continue. Yes would open required text boxs to add:
•the name of the park
•what pavilion
•is power provided
•is water provided
With an additional spot of a small disclosure about an adult remaining with the unit at all times until pick-up. If you agree check here to continue.
