Lead showing duplicates of all items, but totals do not add up and log doesnt reflect the change...

As of 2 days ago, there were only 2 items on the lead (as evidenced by previous emails and the saved contract). Today, a worker noticed there's 2 of each item. However, the log doesnt show that this happened, and the total is still correct for 1 of each. So on the left side of the back-end lead page, there's $1300+ of items, but on the right is the correct amount (pre-duplication)

The only think I noticed was on the Log tab it shows two entries for the customer putting in their tax exemption, but I was unable to reproduce this and have not seen this happen on any other events before.
For now go ahead and delete the duplicate items to fix the event. If it happens again please let me know.
