I'm trying to generate a report sorted by county, but many counties are not filled in?

Just curious why some are and some are not? Is there a setting I am missing?
There are some Cleveland orders labeled as Cuyahoga, some left blank, for example

Can you give us an example lead that is missing a county? We pull the data from MapQuest when we get distances. I wonder if they aren't sending it in some instances or if they are presenting it differently than we expect.
