distance charge added when it shouldn't be?

A customer contacted me and said that she was trying to do a quote and it was adding a $400 distance charge, but she was only one town over.
My settings should not add any distance charge for her town, zip code or distance.
Can you look at the following quote number to see why this profile is adding a distance charge? I created this quote, from her customer profile and it did the same for me.

If you click on the little Google Maps or Mapquest icon next to the address on the lead, you'll see that in both cases they do not like the address. We pull data from those services. If they cannot map the address, you can get strange distances and then charges. It appears to be either a bad address or a new address that is not yet mapped. Other than correcting the address if wrong or contact Google or Mapquest about mapping the address, there is really nothing that can be done. It is not a problem with your IO account. Thanks!
