How can I make a delivery free within 20 mile radius, and then charge $xx.xx per mile thereafter?

In Settings->Distance Charges, You'll have to set your restriction to "none" "state" or "total" at the top.

Then,

Click on the "Distance" tab at the bottom.
Click at the bottom right to "add new" line.
Put "20" in the "miles" box, and "0" in the "charge" box.
Click Save.
Then move to the "Per mile" tab and put your per mile rate in (it will charge this after looking to see if they're within the 20 miles in the distance tab).
