Contact Info + Quote Page

Is there a way we can customize the contact information page. I like to gather more information that just the standard.
Also, for my Quote request page, I noticed when I copy the link and paste it and then click on it. The link takes me to a quote that has all the information already filled out. Anyway to clear that information. I'm having trouble figuring it out myself. Thanks.

Also, viewing the Quote Request page autofills it with whatever lead you were looking at last. For your customers, it will only be filled out if they filled it out before. The information is saved on the computer you're using, not being autofilled by IO.
