Automatic email not sent

We have an automatic email set up (copy of contract for customer when they have booked directly online) that is "active" and set to send immediately ie. "0 days".
There are cases however where these are not being sent? Example and details below - could you check and advise why the email is not being sent.

I also don't understand why this isn't an automatic thing built into IO? By default when a customer books online or even completes a quote, unless we custom create an email to go out, they don't have a copy of what they've completed after they've closed the browser?

An auto email was sent to this customer for a quote...but it was on a different lead.(6588341)But since it was within minutes the system isn't going to keep sending emails over and over again to the same email address
