Aded a question to manager report incorrectlyDefault Setting

I tried to add a last question to the manager report: Did you deliver the attraction before the event start time, and did you call the customer before setting out for delivery. But I could not see how to add a place for a yes, no or description for their answer. When I saved it, this prevented managers from completing their report because the last question could not be answered and the report would not allow them to finish without answering it.
