Automation issue

I have set up an automation to send a text to the client when clients fill out a form and the wp form is working. The automation changes the status on all of my tests but a text hasn't went out but one time on all of my tests.. So let's say 5 tests I get email form filled out emails from my alert emails to staff.. Automation runs and changes status but no text to the number entered..
Can you look into why
BTW I tested this over several days over the weekend
