I like the one that seems to be used frequently.

Accidental Damage Waiver: Would you like to purchase the RESIDENTIAL Equipment Damage Waiver protection plan? For 7% of your rental fee OPTIONAL, you can Get ALL THE FUN without the added worry of accidental damage. We will waive any unintentional damage that happens to our equipment during your rental, excluding intentional damage or theft. Unintentional damage EXCLUDES the following: silly string, gum, paint, face paint, markers or pen, cuts, slices, punctures by sharp objects such as from animals, shoes or sharps and any damage caused by fireworks, firepits, or cigarettes. As a frame of reference on cost, repairs can average anywhere between $100-$500 for items covered by unintentional damage. MOVING THE UNIT VOIDS THE DAMAGE WAIVER* (Recommended)

You already have this on your checkout page the customer has to click on it to see it.

Thank you. Since I have the default as "checked" is there a way to have this as a text box showing the full text next to the check box or under it so the customer see's it without having to click it. If they click it, it will uncheck the box.

I have moved this so that the customer does not have to click to see this.

I see that the formating changed a little when it was added. For example it lost all the comas between the items excluded.

Go to Settings-->Pages/Templates and it's in your WP Checkout page in the code.
