Our web team has found that these fields were due to the text notifications checkbox being selected. We have this removed as it is only necessary on your checkout page.

What form created that lead? Chances are that form had 2 fields that weren't linked to an IO field by changing their IDs. If you go to WordPress and under "Elementor," there's a "submissions" option. Usually, you can go there and determine what those are actually for and then fix the form to link the field to IO fields appropriately.

Good idea...though strangely there's no submission listed for the date the lead was created in IO. I can't say I know how to link the fields even if I found what I was looking for, but I'm not sure how the lead got there in the first place now (other than that the User who created it was "Customer").
