Please add "item duplicated" to lead log

We have had two instances in the last week of items being duplicated on leads. While trying to figure out how it happened, I noticed that the log does not show when items are duplicated. Can you guys add that to the log? It might help me figure out what is happening.

If you look at lead 53372205, maybe you can figure it out. The photo lounge was duplicated and we can't figure out how/when that happened.
