Distance/Mileage not on quote?

I'm 90% sure our mileage is set up correctly - $0.25/mile, with a minimum order or $400 for locations >60 miles away.
However, when I put in quotes, I do not show mileage anywhere. Does the mileage not appear as part of the quote process? I don't even see a field for it.

Should be working now...there was a setting on your delivery method "fully staffed" that needed adjusted so that it would charge the distance charges instead of a flat fee.
