Adding event notes section to Quote Request template?

Is there a way to add the Event Notes section to our quote request form? I'd like to have a place for customers to add any comments/notes we need for the event, such as gate codes. Thanks!

Hello!
I added an optional field to your Quote Request form called Event Notes/Details. I currently have placeholder wording that says "Add Notes Here". If you would like to change the wording, go to Settings-->Optional Fields and select Event Notes/Details.
