Inventory Item showing up 2 times on leads

I have an item Chaffer 8 Quart, it shows up as 2 different items when putting in a lead. one is just the chaffer, the other is the chaffer then with the add on option which i want to keep.
but when i go into inventory it is only listed 1 time there and it has sterno and extra pans listed where the one with extra pans listed in the lead inventory picker doesnt have sterno etc.
Where and how can i fix this

This has something to do w/ the chafer Extra pan 8qt...when I remove it....it works fine. But add it and it goes back to acting weird again? Testing more and may have to go in front of a developer
