partials being created

when a new lead comes in through the quote form, a partial is being created first and then a quote and the partial stays in. Happening on every lead. Example:
Partial: #81297753 (may have been changed to lost, we do that manually)
Lead: #81297845

Do you have any more examples of this happening? I'd like to try to narrow down the issue and try to reproduce it but this single example isn't quite enough for me to get a total picture of what's going on.
