If you go to IO settings you can search for QR code generator and it will allow you to enter an url into the text and it would create a QR code you can use for this

I guess my question is two part. I need to know if there is a blank waiver that isn't connected to leads. I am doing a pay for play and need some kind of waiver for the parents to sign before their kid hops on the inflatable.

No, sorry. We don't have a way to create this within the software. You may want to use an outside source and then use the url.
