Accidental Damage Waiver

I want the damage waiver automatically checked. I looked at my optional field settings and it looks correct. But when I go to make a dummy order, its not checked. Help!

Thank you - I guess it's only doing this when we make an order/quote on the backend. We have to check it each time for it to apply and sometimes forget to look for that.
