In QBO, we use projects (similar to sub-customers) to track income and expenses on a per-lead (not just per-customer) basis. Can the IO-QBO integration be tweaked so that when a transaction is created in QBO, a project is created too?

Unfortunately, that is not an option currently. We are not familiar with projects in QBO.
