Referral Missing From Quotes

Hello! I've always had "How did you hear about us?" with the referrals field on my quote form but somehow it's missing now. I'mn not sure if recent changes made it disappear or what. Regardless, how can I get it back on there? I see it in the WP Checkout code but it's not showing up. Any help on checking settings for this is greatly appreciated!
