I need some guidance creating a Report

I am trying to create a report that will show me each worker's response to the Manager Report that crew leaders are required to send it after completing a delivery. Attachments are to have pictures of the setup and the report followiong the pickup is to answer pertinent questions about the customer, the site and condition of the inflatable. I want my report to show each worker's history of completed or not completed because this report is being skipped too often. I don't need to see the report but do want to see each worker's assigned delivery and whether the report and attachments were completed.
