I wanted to add another question to my quote form, I found what I wanted under template variables so I went to page template then to quote request and added it in. It shows up fine on there when I click the link to preview it but it doesn’t show on my website. Thank you.

I added how did you hear about us and added (optional) next to damage waiver.
