waiver info missing fields

We should probably have a Date of Birth field on the waivers that when filled in on the form, it populates on the signed waiver. I created optional fields, but don't see where they go after they are filled in and signed. My Date of Birth field is just empty on the agreement and I don't see a column in the signed waivers showing what I put in.
