I acknowledge that climbing activities involve inherent risks including, but not limited to, falls, equipment failure, and injury due to my own or others’ negligence. I voluntarily assume all such risks and agree to release and hold harmless the climbing facility, its owners, employees, and agents from any and all liability for injury, death, or damages arising from participation.

I have read and understand the risks involved in climbing activities.

I agree to release the facility and its staff from liability for any injuries sustained.

I have disclosed any medical conditions that may affect my ability to climb safely.

BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY, AND I VOLUNTARILY AGREE TO ITS TERMS.
